1. Show-Me State Games Entry Forms
This is a multipurpose document.
This form includes the name of participants, social security number, home phone, date of birth, sex, sport code, and also the Waiver of Liability and the Consent for Medical Treatment and other personal information related to participants.
Both the Waiver of Liability and Consent for Medical Treatment must be signed in order for an individual to participate in the activities of the Show-Me STATE GAMES.
Authorized Retention:
Retain for twenty-three (23) years from the year of creation, then destroy.

People participating in Missouri Senior Games must be 50 years of age or over.
Both the Waiver of Liability and Consent for Medical Treatment must be signed in order for an individual to participate in the activities of the Show-Me STATE GAMES.
Authorized Retention:
Retain for five (5) years from the year of creation, then destroy.
