1. Veterinary Medical Diagnostic Lab (VMDL) Submission Form
These records consist of forms submitted to VMDL by clients. The documents instruct VMDL as to what tests are to be performed on samples submitted.
Authorized Retention:
Retain for five (5) years, microfilm, and then destroy paper originals.
2. Clinical Pathology Request/Submission & Result Forms
These records consist of forms submitted to Clinical Pathology by the Veterinary Teaching Hospital and clients of VMDL. These forms instruct Clinical Pathology as to what tests are to be performed on the samples submitted. Result forms pertain to the data collected from the testing of the samples.
Authorized Retention:
Retain for five (5) years, microfilm, and then destroy paper originals.
These records are results of culture tests performed on tissue of animals.
Authorized Retention:
Retain for five (5) years and then destroy.
