1. Former Participant Medical Records

These records contain health information for the program participant from their admission to discharge from our program and include nursing notes and records from the physician, PR and marketing files and are retained in the department.

Authorized Retention:

Retain for five (5) years after participation, and destroy as confidential.

2. Student/Community Volunteer Records

These records contain basic contact information, communicable disease testing and criminal background checks for all student/community volunteers within our department.

Authorized Retention:

Retain for administration/reference value, not to exceed five (5) years after last service, and destroy as confidential.

3. Marketing and Public Relations Records

These are documentations of fund-raising efforts, mailings and also include contact information for participants/primary caregivers.

Authorized Retention:

Retain for administration/reference value, not to exceed five (5) years after last service, and destroy as confidential.
